Processing of personal data
In accordance with the Law of Ukraine "On the Protection of Personal Data" for the purpose of maintaining a personal data base and for the purpose of ensuring personnel administration, preparation in accordance with the requirements of the legislation of statistical, administrative and other information on personnel issues, as well as on the implementation of the rights and obligations defined by the law in the field of labor relations, I agree to:
processing of my personal data from primary sources in this volume, in particular, information about education, profession, specialty and qualification, scientific degree, academic title, professional development, passport data, data on awards, information on declaration of income, obligations of a financial nature,
and information about the property status of oneself and members of one's family (for a civil servant and a person applying for the position of a civil servant), autobiographical data, information about work activities, personal information (age, gender, marital status, family composition, etc.) ), information about registered or actual place of residence, information about stay
on the military record, data related to the state of health within the limits defined by the law, regarding the period of granting vacations, regarding confirmation of the right
for the benefits established by the legislation regarding the registration of a natural person
in the State Register of natural persons - taxpayers (taxpayer identification number), electronic identification data (biographical certificates, telephone numbers), image recording (photo);
use of personal data, which involves any actions of the owner of personal data to process such data, their protection, as well as actions to grant partial or full right to process personal data to other subjects of relations related to personal data carried out with consent subject of relations related to personal data carried out
